Open source EHRs

Open source software has gained significant popularity in various industries due to its collaborative nature, flexibility, and cost-effectiveness. In healthcare, open source technology empowers organizations to customize and enhance software solutions according to their specific needs.

Open Source in Healthcare IT allows for rapid development, peer review, and community-driven innovation. Open source EHRs offer increased interoperability, data sharing, and scalability, enabling healthcare providers to deliver better patient care. It fosters an environment where developers feel confident to contribute their ideas, and users can trust in the software to manage their health enterprise. The open-source model serves as a bridge connecting developers, users, vendors, and service providers, enabling efficient interactions and collaborative innovations.

One such initiative was the Open Source Electronic Health Record Alliance (OSEHRA). Established by the US Department of Veterans Affairs (VA) around 2011, OSEHRA aimed to be the cornerstone of open-source software efforts in the health information technology community. It focused on nurturing a vendor-neutral community engaged in advancing EHR software and related health information technology. Through this community, OSEHRA fostered the growth of an ecosystem of complementary products, capabilities, and services, characterized by transparency and agility, reminiscent of open-source software projects.

Among the key products developed under the OSEHRA initiative were the OSEHRA VistA and popHealth. OSEHRA VistA is an open-source EHR that is community-supported, customizable, and can be implemented by developers or providers in the healthcare industry. It maintains compatibility with the VistA EHR system developed by the VA, while also integrating enhancements from the wider community.

The popHealth, on the other hand, is an open-source quality measure reference implementation that aids healthcare providers in meaningful use quality measure reporting and supports easier submission of quality measures to public health organizations.

Success Stories

Several healthcare organizations have successfully implemented OSEHRA and reaped its benefits. For example:

VA VistA: The U.S. Department of Veterans Affairs implemented the Veterans Health Information Systems and Technology Architecture (VistA), an open source EHR solution developed under OSEHRA. VistA has improved patient care, increased efficiency, and reduced healthcare costs within the VA healthcare system.

Indian Health Service: The Indian Health Service (IHS) adopted OSEHRA’s open source EHR solutions to enhance healthcare delivery to Native American populations. The implementation has resulted in improved data sharing, interoperability, and better patient outcomes

Challenges / Limitations

While OSEHRA brings numerous benefits to the healthcare industry, it also faces certain challenges and limitations:

  1. Vendor Lock-In: Transitioning from proprietary EHR systems to OSEHRA may require significant effort and resources. Healthcare organizations must carefully consider the potential vendor lock-in associated with proprietary solutions.
  2. Integration Complexity: Integrating OSEHRA with existing healthcare IT infrastructure can be complex, particularly in large healthcare systems with diverse legacy systems. Robust planning and expertise are required to ensure smooth integration.
  3. Lack of Standardization: Despite efforts to promote interoperability and adherence to standards, variations in data formats and system configurations across different healthcare organizations can hinder seamless data exchange.
  4. Maintenance and Support: OSEHRA implementations require dedicated resources for ongoing maintenance, updates, and technical support. Healthcare organizations must ensure sufficient staff and funding to sustain the system.
  5. User Training and Acceptance: Transitioning healthcare professionals to a new EHR system requires comprehensive training and change management strategies to ensure user acceptance and minimize disruption to workflows.

The shut down

However, the future of OSEHRA became uncertain as the VA and the Department of Defense, the largest users of VistA, decided to replace the system. The organization faced a critical juncture where it had to expand beyond VistA or risk becoming irrelevant. Despite these challenges, OSEHRA continued to maintain and develop the VistA source tree, keeping it open for community contributions.

Unfortunately, OSEHRA shut down in February 2020, with most of its assets, including web content, training materials, and journal content, transferred to WorldVistA. WorldVistA, founded to extend and collaboratively improve the VistA EHR and health information system for use outside its original setting, is continuing the mission of improving healthcare worldwide by making medical information technology better and universally affordable. It seeks to help those who choose to adopt the VistA system to successfully master, install, and maintain the software for their own use, fostering a community based on principles of open, collaborative, peer review software development and dissemination.

The evolution of OSEHRA and its subsequent transition to WorldVistA exemplifies the dynamic and adaptable nature of open-source initiatives. Despite the hurdles, the commitment to drive innovation, ensure freedom from vendor lock-in, lower total cost of ownership, and enhance health information technology interoperability remains steadfast. Open source is indeed the wind of innovation that is constantly shifting the software development business model from selling proprietary software packages to providing software based on vibrant innovation, epitomizing the essence of open-source methodologies.
